Aboveground Petroleum Storage Act (APSA)/Spill Prevention,
Control, and Countermeasure (SPCC) Plan Forms

HAZARDOUS WASTE
-A REQUIRED FORM for all facilities that generate, store, accumulate, consolidate, treat or dispose of hazardous waste.
-A REQUIRED FORM for businesses that routinely generate hazardous waste. IMPORTANT NOTE: This application must be submitted to the State! EPA ID numbers are issued by the Department of Toxic Substances Control .
Temporary EPA ID numbers are issued to a person or business that does not routinely generate hazardous waste.
-A REQUIRED FORM if you recycle onsite more than 100 kg/mo of recyclable material as defined in Section 25143.2 of the California Health and Safety Code.
-A REQUIRED FORM if you are a generator and you collect hazardous waste that is non-RCRA (not federally-regulated) initially at remote sites and subsequently transport the hazardous wastes to consolidation sites which you also operate. For further information, see Section 25121.3 of the California Health and Safety Code .
