Hazardous Waste Generator Permit Application

-A REQUIRED FORM for all facilities that generate, store, accumulate, consolidate, treat or dispose of hazardous waste.

Permanent EPA ID # Application

-A REQUIRED FORM for businesses that routinely generate hazardous waste. IMPORTANT NOTE: This application must be submitted to the State! EPA ID numbers are issued by the Department of Toxic Substances Control .

Temporary EPA ID # Application

Temporary EPA ID numbers are issued to a person or business that does not routinely generate hazardous waste.

Recyclable Materials Report

-A REQUIRED FORM if you recycle onsite more than 100 kg/mo of recyclable material as defined in Section 25143.2 of the California Health and Safety Code.

Remote Waste Consolidation Site Annual Notification

-A REQUIRED FORM if you are a generator and you collect hazardous waste that is non-RCRA (not federally-regulated) initially at remote sites and subsequently transport the hazardous wastes to consolidation sites which you also operate.  For further information, see Section 25121.3 of the California Health and Safety Code .

Underground Storage Tank Form A

-A REQUIRED STATE FORM for all UST sites that designates the owner/operator of the UST site along with information required by the State Water Resources Control Board (SWRCB).

Designated Operator Monthly Visual Inspection Checklist

Certification of Financial Responsibility

Small Quantity Generator Application

-This application is required for any individual or company that generates less than 200 pounds of medical  waste per month and does not treat that waste onsite.

Large Quantity Generator/Small Quantity Generator with Onsite Treatment

-This application is required for any individual or company that generates 200 pounds or more of medical waste in a month.  This application covers Large Quantity Generators with offsite treatment, Large Quntity Generators with onsite treatment and Small Quantity Generators with onsite treatment.

Limited Quantity Hauling Exemption Application

-This application is for any individual or company that generates less than 20 pounds of medical waste a week and transports less than 20 punds of medical waste at one time.
